- Your access to and use of the InterQual® Criteria/Content is limited to reviewing the criteria used by BCBSAZ for utilization management activities related to specific BCBSAZ members.
- You will not disseminate any portion of the InterQual® Criteria/Content in any way.
- Your right to access and use the InterQual® Criteria/Content is non-transferable, nonexclusive, and is for the sole purpose of providing care for BCBSAZ’s members.
- You will access and use the InterQual® Criteria/Content only to the extent necessary to review/evaluate the InterQual® Criteria/Content relevant to your or your employer’s specialty area or related to the care of a specific member's condition.
- You will protect the confidentiality of the information contained in and provided by the InterQual® Criteria/Content with the same care and security you use to protect your or your employer’s own confidential information.
- You acknowledge and agree that any unauthorized disclosure or distribution of the InterQual® Criteria/Content may result in irreparable injury to BCBSAZ and BCBSAZ licensor(s) that will entitle them, individually or collectively, to obtain immediate injunctive relief in addition to any other legal remedies available.
- You will not modify, translate, decompile, disclose, create nor attempt to create any derivative work of the InterQual® Criteria/Content.
- You acknowledge that the InterQual® Criteria/Content is not intended to prescribe, designate or limit medical care to be provided or procedures to be performed.
- You acknowledge that you or your employer will exercise independent judgment in using the InterQual® Criteria/Content and that you or your employer will be solely responsible for such use.
- You agree that you or your employer will indemnify and hold BCBSAZ, and its affiliates, officers, agents, licensors or other partners, and employees, harmless from any claim, demand or damages, including reasonable attorneys' fees, arising out of your or your employer’s use of the InterQual® Criteria/Content or from your or your employer’s violation of the intellectual property rights or confidentiality obligations contained in this agreement.
- You acknowledge that the InterQual® Criteria/Content, including all applicable rights to patents, copyrights, trademarks and trade secrets inherent therein and appurtenant thereto, are the sole and exclusive properties of BCBSAZ’s licensors who have licensed such rights to BCBSAZ.
- You agree that your access to or use of the InterQual® Criteria/Content does not convey to you any rights in or to the InterQual® Criteria/Content.

WARNING! BCBSAZ is aware of data issues with this tool and will resolve these issues ASAP:
1) All designated PCPs may not be available for searches.
2) Search results may be incomplete and likely will not include all attributed PCP-HMO patients.
