To register your organization for an AZ Blue provider portal account, please complete the form below (requires your organization’s tax ID).
If your organization already has a provider portal account but you are unable to access it, there is no need to register again. Simply contact our eSolutions support team at 602-864-4844. They are ready to help you with a password reset or any other portal issues you may be experiencing.

Password Criteria:
Must be a minimum of 8 characters

Must contain 1 lower case letter

Must contain 1 upper case letter
<br />
Must contain 1 number

Cannot include spaces or special characters
<br />
Cannot contain part of username
Must be at least 4 characters
<br />
Cannot be same as password
<br />
Cannot be same as username
<br />
Cannot contain part of security question
